These Terms of Use (“Terms”) govern your access to and use of mazendo.com (the “Site”), operated by Maz Endodontics. By using the Site, you agree to these Terms and to our Privacy Policy. If you do not agree, please do not use the Site.

Everything on this Site, including service descriptions, blog articles, FAQs, and any statements about procedures or outcomes, is provided for general educational purposes only. - No doctor-patient relationship is created by reading the Site, submitting a form, sending a text, or requesting an appointment. A doctor-patient relationship with Dr. Ebrahimi is established only through an actual consultation and examination at our office.

- Individual results vary. Descriptions of procedures, comfort, recovery, and outcomes reflect general information and typical experiences. No outcome is promised or guaranteed for any individual patient. Whether any treatment, including sedation, is appropriate for you can only be determined through an in-person evaluation of your health history and condition.
3. Appointment Requests
Submitting an appointment request through the Site is a request, not a confirmed booking. Appointments are confirmed only when our office contacts you. Information submitted through forms travels as described in our Privacy Policy.
4. Educational Content and Review Dates
Articles on this Site are reviewed for accuracy by Dr. Ebrahimi and carry a review date where shown. Dentistry evolves, and content reflects understanding as of its review date. Content is written for a general audience and cannot account for your individual circumstances.

9. Insurance and Payment Information
Statements on the Site about insurance participation (including that we are in-network with PPO plans and are not contracted with Medi-Cal, Denti-Cal, or HMO plans) are general and current as of their posting. Coverage depends on your specific plan. Verify your benefits with your insurer or with our office before treatment; nothing on the Site is a promise of coverage or a quote of final cost.

Any dispute arising from these Terms or the use of the Site will be brought in the state or federal courts located in Los Angeles County, California, and you consent to their jurisdiction.
