Maz Endodontics is an out of network specialist. Most PPO plans still reimburse a meaningful share of endodontic treatment, and the difference is often smaller than patients expect. You will know the cost before treatment begins.

Maz Endodontics is an out of network specialist, including with the plans below. That does not mean you are not covered. Most PPO plans pay a substantial share of specialist treatment regardless of network status.

Being out of network sounds worse than it usually is. Here is exactly how it works so you can plan.

You are still likely covered

PPO plans are designed to let you see any provider. They typically pay a percentage of specialist treatment whether or not the specialist is contracted with them. Coverage varies by plan, so check your specific benefits with your insurer.

You know the cost before we begin

After Dr. Maz examines the tooth, we tell you exactly what the treatment costs. Payment is due at the time of service, and you will never be surprised by a number after the fact.

We give you everything you need to claim

You submit the claim to your insurer for reimbursement. We provide an itemized receipt with the procedure codes and documentation your plan requires, so the submission is straightforward.

Worth checking before your visit

Call the number on your insurance card and ask three things: what percentage your plan pays for out of network endodontic treatment, what your deductible is, and how much of your annual maximum remains.

Cost

What affects the price of a root canal

No two teeth are the same, so no honest quote comes before an exam. These are the things that actually move the number.

Which tooth

Front teeth have one canal. Molars can have four or more. More canals means more time and more complexity.

First treatment or retreatment

Redoing a failed root canal from another office takes longer than treating a tooth for the first time.

Whether you choose sedation

IV sedation is optional and priced separately. Most patients do fine with local anesthetic alone.

Your plan and where you are in the year

Your deductible, your coverage percentage, and how much of your annual maximum is left all change how much you get reimbursed.

We bill your PPO plan directly and file the claim for you, including the imaging your insurer needs.

Financing that lets you spread treatment costs over monthly payments. Ask us at your visit.

Insurance questions we hear every day

Does Dr. Maz take Delta Dental?

Yes. Maz Endodontics is in network with Delta Dental of California, along with MetLife, Anthem Blue Cross, Blue Shield of California, Guardian, and Cigna.

Do you accept Medi-Cal or Denti-Cal?

No. We are not contracted with Medi-Cal, Denti-Cal, or HMO plans.

What if my plan is not on your list?

Most PPO plans still cover a meaningful share of specialist treatment out of network. Call us with your plan details and we will verify your benefits before you come in.

Do you file the insurance claim for me?

Yes. We submit the claim and the supporting documentation directly to your insurer.

Will I know the cost before treatment starts?

Yes. After we verify your benefits we tell you what your plan is expected to pay and what your share will be, before you commit to anything.

Is IV sedation covered by insurance?

Coverage for sedation varies quite a bit by plan. We check this specifically when we verify your benefits so you know before the day of treatment.
