1. Scope of This Policy

This Privacy Policy describes how Maz Endodontics (“we,” “us,” “our”) collects, uses, discloses, and protects personal information gathered through our website, mazendo.com (the “Site”), and through communications you initiate from the Site, including form submissions, phone calls, and text messages.

This Policy covers website and communications data only. Protected health information (PHI) that we create or receive in the course of providing dental care is governed by our Notice of Privacy Practices, which is a separate document required by federal law (HIPAA). Where information you submit through the Site becomes part of your treatment record, the Notice of Privacy Practices controls.

2. Information We Collect

Information you provide directly:

Appointment and contact forms. Name, phone number, email address, the reason for your visit, and any message you choose to include. Please do not include detailed health history in website forms; that information is better shared during your visit or by phone.
Phone calls and text messages. When you call (310) 248-3636 or text our emergency line at (310) 629-3636, we receive your phone number and the content of your message so we can respond to you.
Reviews and public feedback. If you post a public review, that content is governed by the platform you posted it on (Google, Yelp), not by us.

Information collected automatically:

Usage data. IP address, browser type and version, device type, operating system, referring pages, pages viewed, and dates and times of visits.
Cookies and similar technologies. Small files placed on your device that help the Site function and help us understand how it is used. Our hosting platform (LiteSpeed) sets performance and caching cookies; our analytics tools set measurement cookies.
Analytics. We use Google Analytics 4, implemented through Google Site Kit, to measure Site traffic and understand which pages visitors find useful. Google Analytics collects usage data through cookies and similar identifiers. We use this data in aggregate; we do not use analytics to build individual profiles of visitors. Information we do NOT collect through the Site:

We do not run advertising trackers or sell data to advertisers.
We do not use session-recording tools that capture your keystrokes or screen activity.
We do not knowingly collect information from children under 13 through the Site.

3. How We Use Information

We use the information described above to:

Respond to appointment requests, questions, and emergencies
Schedule, confirm, and follow up on visits
Communicate with you by the method you used to reach us (call, text, or email)
Operate, secure, maintain, and improve the Site
Measure Site performance and understand what content is useful, in aggregate
Comply with legal obligations and enforce our rights

4. Text Messaging (SMS)

If you text our emergency line or provide your mobile number in a form or at our office, we may reply by text for purposes directly related to your inquiry or care, such as responding to an emergency request, confirming an appointment, or following up after a visit. Message frequency varies with your care. Message and data rates may apply. You can stop text communications at any time by replying STOP or by calling our office. We do not send promotional text campaigns, and we do not share your mobile number with third parties for their marketing.

5. How We Share Information

We do not sell personal information, and we do not share it with third parties for their own marketing. We share information only:

With service providers that help us operate: our website hosting provider, our practice communications tools, and our analytics provider, each limited to what is needed to perform their function.
Within your care. Information that becomes part of your dental record may be shared as described in our Notice of Privacy Practices, for example with a referring dentist or your insurance plan.
For legal reasons. When required by law, subpoena, court order, or to protect the safety, rights, or property of our patients, staff, or others.
In a business transition. If the practice is merged, acquired, or sold, information may transfer as part of that transaction, subject to this Policy’s protections and applicable health-privacy law.

6. Data Security

We use reasonable administrative, technical, and physical safeguards appropriate to the sensitivity of the information, including encrypted transmission (HTTPS) for the Site and access limits within our practice. No internet transmission is completely secure; if you prefer, you can always call our office rather than submitting information online.

7. Retention

We keep website inquiry information as long as needed to respond to you and for our records. Information that becomes part of your patient record is retained according to the schedules required for dental records under California law and our record-retention policy. Analytics data is retained according to our Google Analytics settings.

8. Your Choices

Forms: you are never required to use website forms; you can call instead.
Cookies: your browser settings let you refuse or delete cookies. Parts of the Site may not function fully without them.
Analytics: use Google’s opt-out add-on or browser privacy controls.
Texts: reply STOP at any time.
Corrections and deletion: contact us at (310) 248-3636 to correct or delete information you submitted through the Site. Note that information in your patient record is governed by medical-records law and the Notice of Privacy Practices, which give you separate rights of access and amendment.

9. California Privacy Rights

Medical information governed by HIPAA and California’s Confidentiality of Medical Information Act (CMIA) is exempt from the California Consumer Privacy Act (CCPA), and as a dental practice, most personal information we hold falls under those health-privacy laws rather than the CCPA. 10. Do Not Track

Some browsers send “Do Not Track” signals. The Site does not respond to these signals, but because we do not run cross-site advertising trackers, your experience is the same either way.

11. Third-Party Links

The Site links to third-party services such as Google Maps, Google reviews, and our social media pages. Those services have their own privacy practices, and this Policy does not apply to them.
